Created by the mad mind of the Great Black Dragon, Sable, this disease targets creatures of reptilian origin (dragons, kobold, giant lizards, dragonborn, etc).  Over time, the creatures scales harden and it’s mind slowly warps into madness, striking the target with rage and confusion.

When a qualifying target is injured by a creature that is a carrying this disease, the target must make a DC 20 Wisdom saving throw or contract the Bakalian Plague.  The first symptoms begin to show in 1-4 hours. An infected creature’s skin and scales begin to harden and they begin to lose control of their emotions, reducing their Dexterity and Wisdom by 1d3 each.  This reduction lasts until the disease is cured. While under the effects of the disease, the target reduces its Dex and Wis by 1d3 every day, and it’s natural armor bonus increases by 1 as it’s body begins to calcify. Each morning, the target can attempt a DC 20 Wisdom save to resist the calcification effects (Dex and Wis reduction and AC increase) for that day only. A new saving throw is required each morning.
After the disease is cured, the affected ability scores begin to heal naturally at the rate of 1 point per day, for each ability. The increase to natural AC decreases at the rate of 1 point per day, as well as their scales begin to heal and go back to normal.
After the first day, the target also becomes prone to Bakalian Plague Frenzy.

Bakalian Plague Frenzy-An infected creature that becomes the target of a successful attack, whether or not it takes damage, must make a successful Wisdom saving throw (DC 10 + damage taken) or become frenzied. While in this state, the creature suffers from the combined effects of a Confusion spell and the Barbarian Rage base ability. The frenzied state lasts for 3d6 minutes or until the victim is killed or rendered unconscious.

RAGE-You have advantage on Strength checks and Strength saving throws.
When you make a melee weapon attack using Strength, you gain a bonus to the damage roll of +2. You have resistance to bludgeoning, piercing, and slashing damage.
If you are able to cast spells, you can’t cast them or concentrate on them while raging.

The plague is highly resistant to curative magics. Heal, Greater Restoration, Lesser Restoration and similar magics and special abilities fail unless the caster is 18th caster level.
